What does “not flagged” mean?

The IMEI was not flagged on the GSMA Block List at check time. A later report can still change the status.

Can a carrier still block a clean device?

Yes. Some carrier-specific restrictions, including certain billing issues, may not appear on the GSMA Block List.

What should I do if it is flagged?

Do not complete the purchase. Ask the seller to resolve it with the carrier or organisation that placed the flag.
